    A standardized survey developed and published by Raymond C. Rosen and colleagues in 1997 which is used to assess erectile function. This instrument is a self-administered questionnaire with 15 items each with as set of 5 or 6 response options associated with a score of 1-5 or 1-6. (NCI Thesaurus)
