    Short Form 36 Health Survey Acute, US Version 1.0 Questionnaire

    A standardized, multipurpose, general health survey developed by John E. Ware and copyrighted by the Medical Outcomes Trust and Quality Metric Inc. in 1988 and 2002, which is used to assess the patient's functional health and overall well-being. This first version contains 36 questions that refer to the patient's symptoms or status within the past week and can be completed by the patient.
